چکیده

Four patients had a left-sided focal medial temporal lobe onset. Three patients had a right-sided regional medial temporal lobe onset. One patient had a bilateral regional temporal lobe onset with predominant involvement of the left side. Two patients had a leftsided regional medial temporal lobe onset. Nine out of 10 patients had a >50% reduction of interictal spikes during the initial AH-DBS trial period. In one patient who showed very infrequent spiking, seizure frequency that had significantly decreased, was used as a criterion for implantation. One patient did not meet the chronic implantation criterion and underwent a selective amygdalo-hippocampectomy. Nine out of 10 patients were implanted with an internal generator. The mean follow-up in these patients was 16 months (range: 9–25 months). One patient has been free of complex partial seizures (CPS) for 2 years and has been tapered off 2 anti-epileptic drugs (AEDs). Another patient has become seizure-free in the past 9 months; 3/10 patients have a >50% reduction in seizure frequency; 3/4 patients have been taIntroduction

Treatment of mesial temporal lobe epilepsy with amygdalohippocampal stimulation: A case series and review of the literature

Deep brain stimulation (DBS) is being used with increasing frequency for the treatment of mesial temporal lobe epilepsy (MTLE). Here, we report two patients treated with amygdalohippocampal (AH)-DBS for drug-resistant temporal lobe epilepsy. Two patients with temporal lobe epilepsy were admitted to Beijing Sanbo Brain Hospital. The first patient was a 34-year-old male with a 31-year history of ...

A Decade of Experience with Deep Brain Stimulation for patients with Refractory Medial Temporal Lobe epilepsy

In this study, we present long-term results from patients with medial temporal lobe (MTL) epilepsy treated with deep brain stimulation (DBS). Since 2001, 11 patients (8M) with refractory MTL epilepsy underwent MTL DBS. When unilateral DBS failed to decrease seizures by > 90%, a switch to bilateral MTL DBS was proposed. After a mean follow-up of 8.5 years (range: 67-120 months), 6/11 patients ha...

Hippocampal deep brain stimulation in nonlesional refractory mesial temporal lobe epilepsy

PURPOSE To evaluate the efficacy of chronic continuous hippocampal deep brain stimulation (DBS) in nonlesional refractory mesial temporal lobe epilepsy. METHODS Three adult patients with medically intractable epilepsy treated with hippocampal DBS were studied. Two patients underwent invasive recordings with depth stereo-electroencephalography (SEEG) electrodes to localize ictal onset zone pri...

Deep brain stimulation for epilepsy: knowledge gained from experimental animal models.

Since the development of Deep Brain Stimulation (DBS) for Parkinson's Disease, DBS has been suggested as a treatment option for various other neurological disorders. Stimulation of deep brain structures for refractory epilepsy appears to be a safe treatment option with promising results. As research on the evaluation and optimization of DBS for refractory epilepsy may be difficult and unethical...

Deep brain stimulation in patients with refractory temporal lobe epilepsy.

PURPOSE This pilot study prospectively evaluated the efficacy of long-term deep brain stimulation (DBS) in medial temporal lobe (MTL) structures in patients with MTL epilepsy. METHODS Twelve consecutive patients with refractory MTL epilepsy were included in this study. The protocol included invasive video-EEG monitoring for ictal-onset localization and evaluation for subsequent stimulation of...
